Autism Assessment and Diagnosis Using the ADOS-2

The ADOS-2 (Autism Diagnostic Observation Schedule) is a semi-structured observational scale—internationally recognized as a gold standard—for the assessment and diagnosis of Autism Spectrum Disorder (ASD) in individuals across various ages and language development levels.

The 4 Assessment Domains of the ADOS-2 Test

The ADOS-2 test evaluates the patient across four fundamental clinical domains:

  • Communication and language: Assesses how the patient uses verbal and non-verbal language (facial expressions, hand gestures, inferences, eye contact, and conversational coherence).
  • Reciprocal social interaction: Measures the patient's ability to engage in appropriate, two-way interactions characterized by reciprocal social and emotional responses.
  • Imagination and creativity: Analyzes the ability to use objects symbolically, create narratives or symbolic play, and incorporate storylines into role-playing activities.
  • Stereotyped behaviors and restricted interests: Identifies repetitive, routinized behaviors, patterns, or interests that interfere with the patient's normal development and functioning.

Clinical Utility and Diagnostic Advantages

This test is highly valuable in a clinical setting for the following reasons:

  • Individualized diagnosis: It enables the diagnosis of ASD and precisely identifies the specific areas where the patient requires further psychotherapeutic intervention.
  • Detection of comorbidities: It is an effective tool for diagnosing conditions frequently associated with ASD, such as ADHD (Attention Deficit Hyperactivity Disorder), depression, anxiety, or tics.
  • Monitoring progress: It facilitates the recording... ...of symptomatic changes as the child grows and various interventions are implemented.
  • Non-invasive method: Provides rigorous clinical information and illustrates symptom variability through a procedure that is simple to administer and adapted to each developmental stage.

The ADOS-2 test allows us to continuously adjust treatments, psychotherapy, and medical approaches to support the patient and their family at every stage of life.

Overview

Specialized Diagnostic Assessment: ADOS-2 and ADI-R at Clínica Tranquilamente

The ADOS-2 assessment is a benchmark test for evaluating and diagnosing autism and pervasive developmental disorders in individuals across various ages and language development levels. It is an individual assessment lasting between 45 and 80 minutes, depending on the patient's age and characteristics. It is the internationally recognized assessment, considered the gold standard for diagnosing Autism Spectrum Disorder (ASD).

What is the ADOS-2? (Autism Diagnostic Observation Schedule)

The ADOS-2 test is a semi-structured observational scale (standing for Autism Diagnostic Observation Schedule) in which two specially trained clinicians present the patient with a series of tasks, games, role-playing activities, conversations, etc., and subsequently score their reactions and behaviors based on standard parameters ranging from 0 to 3.

  • How it is administered: One of our certified specialist psychiatrists or psychologists introduces games, tasks, or conversation topics tailored to the patient's age and language level (ranging from non-verbal young children to verbally fluent adults). A second specialist acts as an observer to enhance the quality of the observation.
  • What it evaluates: It assesses social communication, reciprocal interaction, imaginative play, and the presence of repetitive behaviors or restricted interests.
  • Duration: The test lasts between 45 and 90 minutes.

What is the ADI-R?

(Autism Diagnostic Interview - Revised)

The ADI-R is an in-depth clinical interview conducted with parents or primary caregivers. It allows us to reconstruct developmental history and understand behavior within the context of daily life.

  • How it is administered: It is a guided dialogue that explores early developmental milestones and current behavior in detail.
  • What it assesses: It examines three key areas: language/communication, reciprocal social interaction, and stereotyped behaviors or interests.
  • Duration: Between 1 and 2 hours.

The perfect synergy for an accurate diagnosis

No single test determines a diagnosis on its own. While the ADI-R provides a retrospective and family-centered perspective, the ADOS-2 offers an objective sample of behavior in real time. At Tranquilamente, our psychiatrists and clinical psychologists integrate both sets of results with the overall clinical history to provide you with a diagnostic report that is rigorous, compassionate, and focused on opening doors to the necessary support.

Main Symptoms

Signs and Indicators Suggesting the Need for an ASD Assessment

We know that noticing differences in development or social interaction can generate concern. Identifying these signs early does not aim to label, but to understand the unique needs of the person to provide them with appropriate support. Assessment with ADOS-2 and ADI-R is recommended in the presence of the following indicators:

In Childhood and School Age

  • Difficulties in communication: Delay in speech acquisition, atypical language use (repetition of phrases or echolalia), or difficulty initiating and maintaining spontaneous conversations.
  • Limited eye contact and non-verbal language: Difficulty holding eye contact, using gestures to point to what they want, or interpreting others' body language.
  • Differentiated social interaction: Preference for solitary play, difficulty making friends, or understanding implicit social rules in group play.
  • Inflexible behavior patterns: Intense distress over minimal changes in routine, unusual attachment to certain objects, or very specific and absorbing interests.
  • Sensory sensitivity: Extreme discomfort or fascination with noises, textures, lights, or flavors.

In Adolescents and Adults

  • Social exhaustion (Social Masking): Constant feeling of having to "act" or consciously effort to fit into social contexts, generating great emotional fatigue.
  • Difficulty interpreting social reciprocity: Challenges in understanding irony, sarcasm, or double meanings, as well as managing empathy according to conventional expectations.
  • Need for rigid structure: Significant anxiety when plans change unexpectedly or when thorough processes are not followed.

Causes and Origins

Why Does Autism Spectrum Disorder Develop?

It is essential to clarify that autism is neither an illness nor a condition caused by parenting. At the Tranquilamente clinic, we promote a modern, neuroscience-informed perspective: ASD is a neurodevelopmental condition with a complex and diverse biological basis.

Factors involved in the development of ASD

  • Genetic Factors: Scientific evidence demonstrates a strong hereditary component. Various genes and genetic interactions influence how neurons connect and communicate with one another.
  • Neurobiological Factors: There are differences in brain structure and function, particularly in areas responsible for social, emotional, and sensory processing.
  • Prenatal and Perinatal Factors: Certain aspects of maternal health during pregnancy or complications during childbirth can interact with genetic predisposition, influencing brain development.

Understanding these causes helps relieve families of unjustified guilt and reaffirms the importance of using scientifically validated tools—such as the ADOS-2 and ADI-R—to understand each individual's neurodivergence.

Common Types

Modalities and Adaptation of ADOS-2 and ADI-R Assessments

The autism spectrum manifests in very diverse ways in each life stage. Therefore, ADOS-2 and ADI-R tools are not applied generically, but strictly adapt to the developmental and linguistic profile of the evaluated person.

ADOS-2 Modules According to Patient Profile

  • Toddler Module (Module T): Designed for young toddlers (between 12 and 30 months) where early socio-communicative development background is evaluated.
  • Module 1: Aimed at children from 31 months who do not use fluid verbal language (from absence of speech to single words).
  • Module 2: For children of any age with phrase speech who are not yet verbally fluent.
  • Module 3: Indicated for children and adolescents with fluent language and ability to play with expressive objects.
  • Module 4: Specifically adapted for older adolescents and adults with fluent verbal language, focused on complex socio-emotional goals, work life, and interpersonal relationships.

Support Levels According to Overall Assessment

Once data from ADOS-2, ADI-R, and clinical evaluation are integrated, required support levels are determined (according to DSM-5):

  • Grade 1: Requires support (profile where social differences or inflexibility cause mild interference).
  • Grade 2: Requires substantial support (more marked difficulties in verbal and non-verbal communication).
  • Grade 3: Requires very substantial support (severe limitations in daily functioning).
